Use this form to change your personal details recorded by the University i.e. change of name. You may change your address details in SOLS. |

On SOLS you can change your address and update your preferred name (for class lists only). Your preferred name will not appear on official documents from UOW.
Returning Your Forms
Completed forms should be returned to the Administration Unit at the Partner Institute you are studying your degree with.
